Personalised care

Wellness Recovery Action Plans (WRAP) is an evidence based self-management tool that supports self-management for people with mental health problems and on evaluation has shown improvements in self-reported symptoms, hopefulness, self-advocacy and physical health66.

Crisis cards have been shown to serve a practical function in enabling a service user to access the most appropriate care, however there is evidence that they also serve a positive function in improving self-stigma, attitudes to treatment, their illness and relationships with staff67.

Joint crisis plans substantially differ from written care plans used standardly with the Care Programme Approach as they are fully agreed and developed between staff and service users: at 6-12 month follow up, 57% of service users felt more involved in their care, 60% more positive about their situation, 51% more in control of their mental illness and 41% more likely to continue in treatment68.
